REPEAT PRESCRIPTIONS

Please complete the  form below to request your repeat prescription medication. This form can only be used for medications which you are prescribed as ‘repeat medication’ on an ongoing basis from us at the practice.

It can take up to 2 working days for your prescription to be issued, though we will process your request as soon as possible. We will notify you when your prescription is ready.

In line with CQC guidelines, we issue a maximum of 3-months of medication per prescription (30 days for controlled drug prescriptions). Patients on repeat medication require regular medication reviews. The frequency of these reviews depend on the type of medication and your medical history. We will inform you when these medication reviews are due.
